Resume and Teaching Experience (10 points)
Attach your resumé or curriculum vitae.* Include all educational institutions, programs, and individuals from whom you have received teaching and/or artistic training such that your educational background and/or training reflect your professional qualifications. Ensure your resumé also includes varied and successful arts-related teaching experiences that demonstrate professional recognition, including dates and locations. (This may include work with schools, libraries, community centers, etc.)

*Arts organizations/ensembles should attach the resumé /cv of current managing director, artistic director, and/or all principal group members who may be involved in residencies, workshops, presentations, etc.
Artistic Skill (20 points)
Share at least three (3) and no more than five (5) examples of work from your art practice. You may include links to your personal website or file-sharing platforms* that demonstrate the quality, depth, and breadth of your artistic portfolio. (This may include photos, videos, sound files, scores, texts, etc.)
*Recommended online platforms that allow photo sharing via a private link include Dropbox, Google Drive, YouTube, Vimeo, Spotify, and Shutterfly.
Teaching Artistry (10 points)
What drew you to teaching? How do you envision yourself as an artist in an educational setting?

The Ohio Teaching Artist Roster promotes the theory and practice of Universal Design for Learning (UDL). UDL tools help “improve and optimize teaching and learning for all people based on scientific insights into how humans learn,” (CAST) and advances understanding that “what’s necessary for some, is good for all.” Briefly describe how you design and execute programming to ensure an experience is accessible for all learners?

Letters of Recommendation (10 points)
Submit two (2) letters of recommendation from individuals* who have first-hand experience with your work as a teaching artist in the past two (2) years. These letters MUST be explicitly written for your application to the Ohio Teaching Artist Roster and should include a recent date. Upload letters here, do not send separately.

*Please note that, due to potential conflicts of interest, Ohio Teaching Artist Roster partner organizations and their staff are not able to submit letters of recommendation for any teaching artists.
Curriculum Materials (30 points)
Use the template linked below to prepare a residency plan of 5 or more sessions. Upload it here along with related lesson plans, study guides, and other support materials you wish to share. NOTE: You are welcome to complete this based on a past program you created, but you must adapt it to the template provided. Refer to the OTAR scoring rubric to assist you in covering important curricular elements.

Instructional Documentation (20 points)
Share a link to a 10-15-minute (no more than 20), unedited video recording of yourself teaching video in a school, community, or virtual/distance learning setting. This can be documentation from a real teaching experience, or a scenario you arrange in order to make this. NOTE: Video submitted will NOT be shared or used for any commercial or promotion purposes without your prior expressed consent. Use the OTAR Scoring Rubric to ensure you include the required instructional elements. URLs for sample videos can be found below.

*To assist applicants in documenting their instruction, partner organizations presented a webinar overviewing teaching artist portfolio development as an integral part of documenting experiences with students and educators. This webinar, a recording of which is embedded below, covers matters ranging from student evaluations and teaching artist philosophy statements to visual elements such as photos and video samples in pursuit of presenting effective and meaningful classroom documentation.
